Exosomes are nano-sized vesicles that cells use to send instructions to one another. Applied to the scalp, they carry proteins, growth factors and microRNAs that signal hair follicles to move back into an active growth phase. The important word is signal. Exosomes do not create new follicles. They speak to the ones you still have.

That single distinction decides whether this treatment is worth your money, and it is the part most clinic marketing leaves out. Below is what the evidence supports, what it costs, and the cases where it is the wrong choice.

01 Can exosomes regrow hair?

Partly, and only in a specific situation. A hair follicle can be dormant, meaning it has shrunk and stopped producing visible hair but is still biologically alive, or it can be gone, meaning the follicle has been replaced by scar tissue. Exosome therapy may wake the first kind. Nothing wakes the second.

In practice that means a scalp with miniaturised hair, visible thinning at the parting, or a widening hairline often has something to work with. A scalp that has been completely smooth and shiny for years usually does not. This is the assessment done before anything is booked, and it is the reason an honest clinic will sometimes tell you not to spend the money.

Androgenetic alopecia deserves a specific note. It is a progressive condition driven by hormones and genetics, so exosome therapy manages it rather than ending it. Sessions maintain a position against an ongoing process. Any clinic presenting a one-off treatment as a permanent fix for pattern hair loss is overselling it.

03 Do exosomes for hair actually work?

This deserves a straight answer rather than a sales one. The laboratory science is genuinely interesting: exosome signalling is an active area of research in hair follicle biology, and reviews of the mechanism have been published in the peer-reviewed literature, including work on the roles of exosomes in regulating hair follicle growth.

What does not yet exist is the thing that would settle the question: large, long-term, independent, randomised trials in humans comparing exosome therapy against placebo and against established treatments. Most of what is published is laboratory work, small studies, or clinic-reported outcomes.

So the honest position is this. Exosome therapy is a reasonable option with encouraging early evidence, not a proven cure. Many patients at the clinic report reduced shedding and better density. Some do not respond as hoped. You should be told which category your scalp is likely to fall into before you pay, and you should treat any guarantee of regrowth as a warning sign about the clinic rather than a promise about the treatment.

04 What is the dark side of exosomes?

Patients ask this often, and it is a fair question. There are three real concerns, and none of them is usually mentioned in an advertisement.

  • The product market is poorly policed. Counterfeit and under-dosed exosome product is a genuine problem in Pakistan. A vial that claims 50 billion may contain a fraction of that, and you cannot tell by looking. This is why every vial at the clinic is unpacked in front of you before it is drawn up.
  • Long-term human data is limited. Regulators including the US FDA have issued public warnings about unapproved exosome products marketed directly to patients. That is a caution about an unregulated supply chain and unproven claims rather than a finding that exosomes are dangerous, but you are entitled to know it exists.
  • The cost is high relative to the alternatives. For a patient with early thinning and healthy platelets, PRP may deliver a similar result for considerably less. Paying more does not automatically buy a better outcome.

In trained hands, with verified product and a proper assessment first, exosome therapy has a mild side-effect profile. The risks worth managing are commercial ones as much as clinical ones.

05 Exosomes or PRP for hair?

This is the decision most patients are actually trying to make, and neither answer is right for everyone. PRP concentrates platelets from your own blood, so its strength varies with your platelet count, age and health on the day. Exosomes are lab-cultured to a known, consistent dose every time.

No brand is best for every scalp. A higher billion count is not automatically the right purchase, and the honest question at consultation is which strength your pattern actually warrants. 07 What a session involves, and the timeline

The scalp is assessed, topical anaesthetic is applied for comfort, and the exosome preparation is delivered through micro-needling, which creates controlled micro-channels so it reaches follicle depth. Fine direct injections may be used in specific areas. Expect mild redness and warmth for 12 to 24 hours.

  • Weeks 1 to 4. Little visible change. Some patients notice reduced shedding when washing or combing.
  • Weeks 4 to 8. Early fine regrowth at the hairline and parting for those who respond.
  • Weeks 8 to 12. The fullest effect of the session as follicle activity peaks.
  • Months 9 to 12. Benefits are commonly reported to hold around this long before maintenance is considered.

Aftercare is simple: leave the scalp unwashed for the first 24 hours, avoid direct sun, and attend follow-ups as advised. How long the effect holds varies with the strength used, your age, your general health and whether the underlying cause is still active. Individual results vary.

08 Getting assessed in Islamabad and Rawalpindi
